Why Does Case Diameter Never Give You A Box Size?
Case diameter measures the round part of the watch while lug to lug measures the full length across the horns that carry the strap.

Two watches sharing the same 42 mm case can still need different boxes because their lugs run 51 mm and 52 mm. Collector watches sit between 46 mm and 52 mm lug to lug across most of the market. A box cut from the diameter alone arrives short at both ends of the strap.
How Wide Does The Slot Around The Pillow Have To Be?
The pillow holds the strap in a closed circle and the slot is the clear space cut around that pillow. Slot width follows the lug to lug figure rather than the case.

How Much Height Does The Lid Need Above The Crystal?
Case thickness is a separate measurement because the lid closes onto the crystal rather than onto the strap. Measure from the case back to the highest point of the crystal, with the bezel counted in.
A dress watch runs under 12 mm thick while a dive watch reaches 14 to 16 mm once the bezel and crystal are included. Add 3 to 4 mm of clear space above that figure so the lid never rests on the glass. Wall thickness then pushes the external size outward because rigid board adds three to six millimeters to every internal dimension.
Does A Steel Bracelet Need A Different Pillow Than Leather?
A leather strap compresses around the pillow while a steel bracelet holds its shape and will not close past its links. The same watch therefore needs two pillow diameters depending on what it ships on.
A thick dive bracelet adds 8 to 10 mm of bulk around the pillow and a heavy rubber strap stands about 5 mm tall at the buckle. The watch should slide on with gentle resistance and it should never need force. Brands selling one model on two strap options order two pillow sizes against the same box.
